Position Overview

The Clinical Liaison for Hospice Services acts as the primary contact for patients and their families entering hospice and palliative care. This role involves educating families about hospice options and providing continuous support throughout the process. The Clinical Liaison is responsible for triaging incoming referrals and coordinating with interdisciplinary team members to address the immediate needs of patients. Collaborating with patients, families, and physicians, the Clinical Liaison assists in formulating a care plan that aligns with the patients' preferences. Additionally, this position initiates assessments to ensure the care plan is suitable and adhered to.

Work Schedule: Monday to Friday, 8:30 AM to 5:00 PM, with occasional weekend shifts

Work Environment: Healthcare Facility

Note: Completion of COVID-19 vaccination is mandatory prior to commencement

Key Responsibilities

The Clinical Liaison for Hospice Services initiates care and discharge planning while maintaining communication with the interdisciplinary team regarding patient objectives and treatment strategies. This role involves making nursing assessments, judgments, and treatment decisions based on the unique needs and desires of patients. The Clinical Liaison promotes the programs of Capital Caring and Capital Palliative Care Consultants within acute care settings by fostering collaborative relationships and offering opportunities for professional growth and community education. There may be a requirement to rotate among different regions or departments.

Qualifications

Experience Requirements

A minimum of one year of experience in a similar healthcare setting is required. Experience in hospice and/or palliative care is highly preferred.

Educational Requirements

A degree in Nursing from an accredited institution is essential. A Bachelor of Science in Nursing (BSN) is preferred.

Licensure Requirements

Must possess valid licensure from the appropriate governing body for practice in the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, depending on the jurisdiction of practice. Licensure from a compact state is acceptable.

Driver's License

A valid driver's license in good standing and access to a vehicle during work hours are required.
