As a Palliative Care Nurse Practitioner, you will play a crucial role in delivering tailored palliative care to Gentiva patients, ensuring that each treatment plan is comprehensive and individualized. You will demonstrate exceptional clinical expertise and ethical judgment while providing diagnosis, treatment, consultation, and follow-up care under the guidance of the Palliative Medical Director, as required by state regulations.


Key Responsibilities:

- Collaborate with the Palliative Care Medical Director and team members to provide holistic palliative care, including diagnosis, treatment, and follow-up for referred patients.

- Assume responsibility for the continuum of palliative care from initial assessment through to discharge.

Assessment Focus:

- Evaluate disease-specific changes, functional decline, nutritional status, cognitive function, pain management, and the overall impact of the disease on the patient's quality of life.

- Develop a personalized palliative care plan in consultation with the patient, family, and Palliative Care Team members. - Facilitate access to necessary care as identified in the care plan.
- Participate in on-call services as required, ensuring clinical competence and responsiveness.
- Prescribe medications, including controlled substances, in accordance with state law and professional practice standards. Order and interpret diagnostic tests and treatments as necessary.
- Maintain regular communication with the Palliative Medical Director regarding clinical findings and care progress.

Ongoing Evaluation:

- Regularly reassess the effectiveness of the care plan and make adjustments as needed.

- Foster effective working relationships with the supportive care team, including Social Workers and Spiritual Care Coordinators, to achieve optimal patient outcomes. - Document clinical findings, treatment plans, and care provided in the patient's medical record, adhering to accepted standards for medical practice and reimbursement.

Qualifications:

Essential Skills:
- Strong leadership, interpersonal, and communication skills, both written and verbal. - Ability to make independent decisions and demonstrate self-motivation.

Knowledge Requirements:

- Understanding of Palliative Medicine principles, including criteria for home health or hospice care, Medicare/Medicaid regulations, and interdisciplinary collaboration.

- Compliance with continuing education requirements set by Gentiva and licensing boards. - Proficiency in utilizing point-of-care software.

Educational Background:
- A Master's degree in Nursing from an accredited institution or an equivalent qualification as permitted by state law. - A minimum of two years of experience as a Nurse Practitioner, preferably in Family or Adult Medicine. - Experience with Part B billing and the ability to work autonomously in an advanced practice role.

Licenses and Certifications:
- Current registered nurse license or equivalent as mandated by state law for Nurse Practitioner licensure. - Valid Nurse Practitioner certification/license. - Collaborative agreement with a supervising physician or agreement to establish one with the Palliative Medical Director, as required by state law. - Federal DEA license and state DEA license (if applicable). - ACHPN certification in Hospice and Palliative Care is preferred. - Valid automobile insurance and driver's license.
