Western Illinois Home Health Care is a local, family owned and operated company seeking a talented social worker to serve communities in West Central Illinois, with a passion to enhance the quality of life of elderly patients to help them safely remain at home. Western Illinois Home Health Care has been serving the community for over 40 years. We are passionate about our mission and are looking for team members who want to make a difference in the lives of others.

Our core hours are Monday through Friday, 8A-5P.

Primary duties of this position include:

Explaining agency services and programs to prospective clients and their families

Developing a tailored care plan to meet the client's individual needs

Helping clients and their families to navigate the healthcare system to ensure their care needs are met

Matching caregivers to clients

Working with assigned caregivers to ensure that the client's needs are met

Providing support and guidance to other staff

Connecting clients and patients to community resources

Conducting in-person visits to ensure the client's physical, mental, and social needs are met

Qualifications:

Bachelor's degree in social work or related field

Minimum of 2 years experience in a professional setting
