Job Title: Community Care Navigator

Center for Alternative Sentencing and Employment Services (CASES) is seeking a Community Care Navigator to provide overall responsibility and accountability for managing and coordinating all aspects of an assigned member's care planning.

Job Summary:

The Community Care Navigator will track and arrange appointments, educate members, and coordinate other aspects of the member's health and community services. This position requires strong communication and organizational skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:
  • Complete all documentation, including assessments, plans of care, and encounters, within required time frames in the web-based health home portal.
  • Provide core care management services, including motivational interviewing techniques to support consumers in addressing their problems.
  • Assure that members have access to and engage in needed services as defined in the member's Care Plan.
  • Meet required levels of case management contacts, including a minimum of 4 per month per member, with 2 of the 4 contacts being face-to-face with the member.
  • Assist members by escorting them to appointments, including medical, behavioral health, and entitlements.
  • Involve family and significant others in the consumer's care planning.
  • Respond to crisis situations as needed.
  • Use available databases and their reporting functions to conduct job duties.
  • Keep abreast of current city, state, and federal regulations, laws, and initiatives pertaining to health home care management and Medicaid redesign.
Requirements:
  • High School Diploma and CASAC certification, or Bachelor's degree with 4 years of social services experience, or Bachelor's degree and NYS teacher's certificate, or Bachelor's degree and NYS licensure as a Registered Nurse, or Master's degree with 2 years of social services experience.
  • Strong knowledge of wellness, recovery, and self-help.
  • Effective communication and written skills are essential.
  • Proficiency with computer and databases.
Preferred Qualifications:
  • Bilingual skills in Spanish highly preferred.
