The Role

We are seeking a highly skilled Care Navigation Coordinator to join our team. As a key member of our outreach and support team, you will play a vital role in providing tailored assistance to clients navigating the complex healthcare system. Your responsibilities will include contributing to care management program development, conducting assessments of patients' needs, and developing individualized care plans in collaboration with patients, healthcare providers, and other stakeholders.

Key Responsibilities
  • Perform high-volume outreach, engaging patients through telephonic and digital methods.
  • Develop and manage a panel of up to 250 members, conducting regular assessments and care planning.
  • Provide ongoing support and guidance to patients in accessing healthcare services, scheduling appointments, and navigating the healthcare system.
  • Coordinate with community resources and social service agencies to address patients' non-medical needs.
  • Advocate for patients' rights and preferences within the healthcare system, ensuring culturally competent and patient-centered care.
  • Maintain accurate and up-to-date documentation of patient interactions, assessments, and care plans.
  • Monitor patients' progress and outcomes, adjusting care plans as needed to achieve desired goals.
Requirements
  • Associate's degree in social work, public health, nursing, or a related field, or 4 years of experience in a similar role.
  • Previous experience working in healthcare, social services, or community outreach.
  • Knowledge of healthcare systems, resources, and community services in the state of New York (preferred).
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust with clients.
  • Cultural competence and sensitivity to the diverse needs and backgrounds of clients.
