Job Summary

ChristianaCare is seeking a highly skilled Nurse Navigator to join our inpatient Supportive and Palliative Care team. As a key member of our team, you will play a vital role in navigating and facilitating the care of patients and their families seeking palliative care.

Key Responsibilities
  • Coordinate referrals from within and outside of Christiana Health Care, ensuring seamless transitions and optimal patient outcomes.
  • Conduct thorough reviews of patients upon initial encounter and periodically throughout navigation, identifying unmet needs and matching them with appropriate services.
  • Assist with identifying potential and realized barriers to care, including transportation, caregiver support, housing, language, culture, literacy, role disparity, psychosocial, employment, financial, and insurance-related issues.
  • Schedule and facilitate scheduling of appointments, ensuring timely and efficient care delivery.
  • Triage family/caregiver phone calls and messages, determining disposition and coordinating response with Palliative clinicians.
  • Organize inpatient palliative census and daily workload, evaluating and triaging consults in collaboration with the hospital team.
  • Support established inpatients in lieu of Palliative provider consult, ensuring continuity of care.
  • Collaborate with the outpatient team for seamless transition, promoting optimal patient outcomes.
  • Provide psychosocial support to patients, families, and caregivers, facilitating referrals and addressing high emotional stress and anxiety.
  • Support discussions with patients and caregivers related to goals of care and end-of-life treatment decisions, facilitating scheduling of family/provider meetings.
  • Apply knowledge of insurance processes, including Medicare, Medicaid, and third-party payers, to establish appropriate referrals and ensure optimal patient care.
  • Monitor progress in achieving defined patient-specific, clinical outcomes through performance improvement activities and outcomes.
  • Manage care flow and navigate multidisciplinary treatment needs, including communication with and coordination of tests and imaging.
Education and Experience Requirements
  • BSN required; Masters in nursing preferred.
  • RN license or eligible for licensure in the state of Delaware.
  • Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferred.
  • Nursing Cert - Certified Hospice and Palliative Nurse (CHPN) - Hospice & Palliative Credentialing Center within 3 years of hire.
Special Requirements
  • RN license or eligible for licensure in the state of Delaware.
