About the Role

We are seeking a highly skilled and compassionate Care Navigator to join our team at Tembo Health. As a key member of our multidisciplinary team, you will play a critical role in supporting caregivers, families, and seniors with dementia to enhance their quality of life and ensure their safety in the home.

Responsibilities
  • Act as the primary point of contact for caregivers, families, and seniors with dementia, providing education and support around dementia, symptoms, caregiving, and what to expect.
  • Screen for unmet care needs, including clinical or medication issues, behavioral issues, safety risks, and psychosocial well-being of both caregiver and patient.
  • Develop personalized care plans with support from the clinical team, identifying goals and implementing strategies to address identified needs.
  • Research and connect caregivers, families, and seniors with dementia to local community resources, ensuring access to necessary support and services.
  • Assist with medication management, reconciling medications and developing strategies to improve adherence.
  • Coordinate medical care, assisting with referrals and gathering and compiling up-to-date medical records.
  • Consult with and triage complex issues with our multidisciplinary team, including licensed clinical team members.
  • Develop and maintain content for informational resources, ensuring accuracy and relevance.
  • Provide documentation and support general administrative tasks, including managing records and handling correspondence.
Requirements
  • College degree in social work or psychology.
  • Comfortable with technology and performing this role virtually.
  • Fluent in English and Spanish with excellent communication skills.
  • Able to work in a multidisciplinary team environment.
  • Experience building rapport and collaborative relationships with patients and families.
  • Able to effectively manage difficult patient interactions with empathy and professionalism.
Bonus Qualifications
  • Master of Social Work (MSW).
  • Experience in healthcare, ideally with dementia.
  • Located in the New York City or Boston area.
