Primary Care Nurse Navigator

ChristianaCare is seeking a skilled Registered Nurse (RN) to join our inpatient Supportive and Palliative Care team as a Nurse Navigator. This full-time role will involve navigating and facilitating the care of patients and their families seeking palliative care in collaboration with the multi-disciplinary care team.
Key Responsibilities:- Coordinate care and assist Palliative Care staff with referrals from within and outside of Christiana Health Care.
- Identify and facilitate referrals for potential and realized barriers to care, such as transportation, caregiver support, housing, language, culture, literacy, role disparity, psychosocial, employment, financial, and insurance.
- Triage family/caregiver phone calls and messages, determining the disposition of calls and evaluating and triaging consults by collaboration with the hospital team.
- Support discussions with patients and caregivers related to goals of care and end-of-life treatment decisions, including hospice care.
- Monitor progress in achieving defined patient-specific, clinical outcomes through performance improvement activities and outcomes.
- Manage care flow and navigate multidisciplinary treatment needs, including communication with and coordination of tests and imaging.
- Masters in nursing preferred.
- Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferred.
- Nursing Cert - Certified Hospice and Palliative Nurse (CHPN) - Hospice & Palliative Credentialing Center within 3 years of hire.
