Key Responsibilities- Provide direct care to patients with renal-related conditions, including IV therapy and medication administration.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current LPN license in the state of practice.
- Basic Life Support (BLS) Certification from the American Heart Association.
- IV Therapy Credentials (preferred).
- Excellent organizational and time management skills.
- Good interpersonal and communication skills.
- Ability to work in a fast-paced environment with minimal supervision.
