About the Role

We are seeking a highly motivated and detail-oriented LPN Care Coordinator to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and experience.

Key Responsibilities
  • Oversee the disease registry database to ensure accurate and up-to-date information.
  • Conduct pre-visit chart reviews to identify patient needs and develop personalized care plans.
  • Coordinate care across the continuum, collaborating with healthcare providers and teams to ensure seamless transitions and optimal patient outcomes.
  • Participate in quality improvement activities to drive excellence in patient care and service.
  • Perform other related duties as required to support the healthcare team.
Requirements
  • Graduate of an accredited school of practical nursing.
  • 2 years of ambulatory care experience, with a preference for 3 years of experience in ambulatory care and home health care.
  • Current licensed practical nurse (LPN) license in the state of practice.
