Job Summary

We are seeking an experienced Support Services Coordinator to join our team at Pathlight Group. In this role, you will provide critical support services to families with members who have disabilities, connecting them with community resources and promoting inclusive practices.

As a Support Services Coordinator, you will work closely with families to develop personalized support plans, provide training and education, and facilitate connections with local resources and organizations.

Key Responsibilities:

  • Develop and implement individualized support plans for families, based on their unique needs and goals.
  • Provide training and education on disability support services, inclusive practices, and community resources.
  • Facilitate connections between families and local resources, including healthcare providers, educators, and community organizations.
  • Collaborate with other professionals to provide comprehensive support services.

Qualifications:

  • Bachelor's degree in a related field or equivalent experience working with families and individuals with disabilities.
  • Over 1 year of direct experience in social work, case management, or a related field.
  • Excellent communication and interpersonal skills, with the ability to build trust with families and stakeholders.
  • Valid driver's license and reliable transportation required.

Compensation:

The salary for this position is $41,454.40 annually, plus a comprehensive benefits package.
