About the Role

We are looking for a highly motivated and compassionate Social Worker to join our team at Cooley Dickinson Health Care, a member of the Mass General Brigham system.

As a Social Worker, you will be responsible for providing high-quality social services to patients in their homes, working closely with healthcare teams to ensure comprehensive care delivery.

Salary: $95,000 - $115,000 per year based on experience

You will be part of a dynamic team that values collaboration, innovation, and excellence in patient care.

Key Responsibilities
  • Assess patient needs and develop plans of care to address social and emotional factors impacting patient outcomes
  • Provide education and support to patients and families to promote self-care and independence
  • Collaborate with healthcare teams to ensure seamless care delivery
Qualifications
  • Master's degree in social work from an accredited program required
  • Current LCSW licensure required
  • Minimum one year experience in a healthcare setting required; home care experience preferred
