About the Role

This position involves working closely with families to understand their unique needs and develop personalized support plans. You will serve as a liaison between programs, families, and providers, ensuring seamless communication and coordination.

Key Responsibilities
  • Provide individualized support to families, addressing their specific challenges and concerns.
  • Act as a bridge between programs, families, and providers, fostering collaboration and effective communication.
  • Conduct thorough assessments to identify family needs and develop tailored support plans.
  • Promote community integration by connecting families with local resources and services.
  • Maintain a network of support and informational resources, continually updating and expanding it to meet evolving family needs.
  • Offer case management to families facing complex situations, providing guidance and support to navigate available resources.
Requirements

To succeed in this role, you will require:

  • A broad knowledge base, equivalent to a 2-year college education, encompassing a discipline relevant to disability support.
  • Over one year of direct experience working with families or individuals with disabilities.
  • In-depth knowledge of disability support services, including those provided by Pathlight Group.
  • The ability to complete specialized training, such as First Aid, CPR, OSHA, and Safety Care, as needed.
  • A valid driver's license and reliable transportation/vehicle for occasional travel.
Working Conditions

This full-time position requires a 40-hour workweek, with regular hours from 9 AM to 5 PM, Monday through Friday. Some flexibility may be necessary to accommodate programmatic needs, including occasional evening and weekend shifts.

The annual salary for this role is $41,454.40, offering a competitive compensation package for your skills and experience.
