About the Role:
UnitedHealth Group Inc is seeking a skilled Registered Nurse to fill the position of Senior Healthcare Navigator RN. This role involves providing comprehensive care coordination services to Medicaid and Medicare members with complex medical needs.

Job Description:
We are looking for an experienced Registered Nurse who can work in both field-based and telecommute settings, coordinating care for individuals with complex needs. The ideal candidate will have 4+ years of relevant clinical experience, 3+ years of managing needs of complex populations, and 1+ year of community case management experience.

Key Responsibilities:
* Serve as primary care manager for members with complex medical needs
* Engage members through various modalities to complete comprehensive needs assessments
* Develop and implement person-centered care plans to address member needs
* Partner with internal care teams, providers, and community resources to implement care plans
