Job Summary

We are seeking a skilled Hospice Admissions Nurse to join our team at Visiting Nurse Association of Texas. As a key member of our interdisciplinary team, you will be responsible for providing high-quality nursing care to patients in their place of residence.

Key Responsibilities
  • Assess patients' physical, psycho-social, and environmental needs using the Nursing Assessment and LCD criteria
  • Develop and document individualized plans of care in collaboration with the interdisciplinary team
  • Provide nursing care utilizing infection control measures and work with the team to ensure patient/family needs are met
  • Communicate effectively with attending physicians, hospice physicians, and other healthcare professionals
  • Timely submission of all required paperwork and meet productivity standards
Requirements
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current Texas State license as a Registered Nurse and current Texas Driver's License
  • Two years' experience as a Registered Nurse in a clinical care setting and 1 year full-time hospice experience
  • Reliable transportation and valid and current auto liability insurance
Working Conditions

Works in patients' homes and facilities in various conditions and locations, with possible exposure to blood and bodily fluids and infectious diseases. Must be able to work flexible schedules, travel locally, and be willing to attend to admissions as needed.

Physical and Mental Effort

Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition.
