We are seeking a compassionate and skilled Registered Nurse to join our Inpatient Hospice team at Faith Presbyterian Hospice. As a RN Inpatient Care Specialist, you will provide high-quality patient care, coordinate with interdisciplinary teams, and ensure the best possible outcomes for our patients and their families.
Key Responsibilities- Assume primary responsibility for patient admissions, including assessments and reporting to the team
- Participate in the coordination of patient care, adhering to current professional standards and practice
- Communicate effectively with attending physicians, hospice staff, and other agencies to ensure optimal care and resource utilization
- Develop and evaluate patient care plans, incorporating physical, psychological, social, and spiritual factors
- Maintain accurate and up-to-date patient records, reflecting changes and progress
- Provide emotional support and compassionate care to patients and families, respecting their rights and autonomy
- Collaborate with the interdisciplinary team to seek input and support in patient care planning
- Assist other nurses and assume primary responsibility for patients as needed
- Attend and participate in orientation, in-services, and staff meetings to maintain knowledge and skills
- Provide support at the time of death, reporting to appropriate staff
- Minimum of two years of varied work experience as a professional nurse, preferably in an hospice inpatient setting
- Valid Texas Registered Nurse License
- Proficient ability to communicate orally and in writing in the English language
- Valid Texas Drivers License and reliable method of transportation
- Ability to work independently, adjust to changing conditions, and meet new and/or stressful situations with appropriate responses
- Genuine caring for and interest in the care of mature adults, handicapped people, and hospice patients
