The Healthcare Manager plays a crucial role in assisting patients with complex health issues in navigating the healthcare landscape. This position focuses on empowering patients by serving as an educator, resource, and advocate for both patients and their families, aiming to enhance their quality of life. The Healthcare Manager collaborates with a diverse team of healthcare professionals, including physicians, nurses, pharmacists, and social workers, to ensure comprehensive care. This role requires a self-directed approach within a dynamic environment, adhering to the responsibilities outlined by the RN license in the respective state.

Key Responsibilities:

  • Collaborate with healthcare providers and clinic personnel to identify patients at elevated risk.
  • Facilitate ongoing care for patients with chronic conditions by: assessing health statuses, reconciling medications, conducting motivational interviewing, setting self-management goals, and providing educational resources.
  • Assist in the transition of patients with chronic conditions from hospital to outpatient care by: assessing transitional needs, reconciling medications, developing contingency plans, educating patients, and aiding with post-discharge requirements such as prescriptions and appointments.
  • Work with providers to create or revise care plans.
  • Ensure accurate and timely documentation in electronic health records.
  • Engage in daily huddles and Patient Care Coordination meetings.
  • Prepare and present timely reports for weekly discussions.
  • Maintain professional competence in nursing practices and stay updated on current evidence-based methodologies.
  • May perform clinical tasks within the scope of practice.
  • Carry out other related duties as assigned.

Required Qualifications:

  • Bachelor's degree in Nursing or an Associate's degree in Nursing with a minimum of 4 years of relevant experience.
  • Registered Nurse with an active and unrestricted license in the state of employment.
  • Current BLS certification.
  • At least 2 years of experience in a clinical or hospital environment.
  • Familiarity with chronic diseases, particularly COPD/asthma, diabetes, CHF, and IHD.
  • Strong verbal and written communication skills.
  • Excellent interpersonal abilities.
  • Capacity to engage effectively with individuals and multidisciplinary teams.
  • Proficient in using electronic medical records.
  • Outstanding organizational and prioritization skills.
  • This position requires Tuberculosis screening and proof of immunity to certain diseases.

Preferred Qualifications:

  • Experience in patient education and motivational interviewing techniques.
  • Fluency in both written and spoken English and Spanish.
