The Healthcare Services Supervisor plays a vital role in assisting patients with complex health challenges to navigate the healthcare landscape. This position focuses on empowering patients by serving as an educator, resource, and advocate for them and their families, ensuring they maintain an optimal quality of life. The Healthcare Services Supervisor collaborates with a diverse team of healthcare professionals, including physicians, nurses, pharmacists, social workers, and other specialists. Additionally, this role serves as a support resource for clinic personnel and operates in a dynamic, self-directed environment, performing all nursing responsibilities within the scope of a Registered Nurse license as mandated by the relevant state nursing board.

Key Responsibilities:

  • Collaborate with healthcare providers and clinic staff to identify patients at elevated risk.
  • Facilitate ongoing care for patients with chronic conditions by: assessing health status, reconciling medications, conducting motivational interviewing, setting self-management goals, and providing educational resources.
  • Assist in the transition of patients with chronic conditions from inpatient to outpatient care by: assessing transitional needs, reconciling medications, establishing contingency plans, providing patient education, and aiding with post-discharge requirements such as prescriptions and appointments.
  • Coordinate with healthcare providers to create or update care plans.
  • Ensure accurate and timely documentation in electronic medical records.
  • Engage in daily huddles and Patient Care Coordination meetings.
  • Prepare and submit timely reports for weekly meetings as required.
  • Maintain professional competence in nursing practices and stay informed about current evidence-based methodologies.
  • May perform clinical tasks within the scope of practice.
  • Carry out other related duties as assigned.

Required Qualifications:

  • Bachelor's degree in Nursing or an Associate's degree in Nursing with a minimum of 4 years of relevant experience.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • At least 2 years of experience in a clinical or hospital setting.
  • Knowledge of chronic illnesses, particularly COPD/asthma, diabetes, CHF, and IHD.
  • Exceptional verbal and written communication skills.
  • Strong interpersonal abilities.
  • Capacity to work effectively with individuals and multidisciplinary teams.
  • Proficient in using electronic medical records.
  • Excellent organizational and prioritization skills.
  • This position requires Tuberculosis screening and proof of immunity to specific diseases through lab confirmation, vaccination documentation, or a physician's diagnosis.

Preferred Qualifications:

  • Experience in patient education and motivational interviewing techniques.
  • Fluent in both written and spoken English and Spanish.
