Patient Care Coordinator (Registered Nurse/RN)

The RN Patient Care Coordinator plays a crucial role in managing the comprehensive care and services for patients. This position emphasizes quality care through a collaborative approach that coordinates, monitors, and assesses services tailored to the individual needs of patients.

Collaborating closely with patients and their families, the care team, insurance providers, and external organizations, the coordinator ensures a seamless transition from one care setting to another, adhering to all relevant laws, regulations, and standards set forth by Life Care Centers of America.

Qualifications:
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Minimum of one (1) year of clinical experience in a post-acute care environment is preferred
  • Experience in case management, utilization review, and discharge planning is advantageous
  • Certification as a Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) is preferred
Expertise:
  • In-depth knowledge of practices, procedures, and regulations governing operations in post-acute care facilities
  • Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Proficient in all competency areas, including patient rights, safety, and sanitation
  • Maintain confidentiality of all proprietary and sensitive information
  • Understand and adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing necessary compliance training
Technical Skills:
  • Efficient use of relevant computer systems, including data entry and retrieval
  • Act as a liaison between external case managers, families, physicians, and community resources
  • Provide training and education to patients, families, associates, and other care providers
  • Implement standards of practice for care management, ethical performance, and coordination of care
  • Proficient in reading, writing, speaking, and understanding the English language
