Key Responsibilities- Coordinate Patient Care Progression: Work with patients, families, and healthcare teams to develop and implement individualized care plans, ensuring timely and efficient progression through the continuum of care.
- Discharge Planning and Coordination: Identify and address barriers to discharge, coordinate post-acute care services, and ensure a smooth transition to the next level of care.
- Patient Advocacy: Act as a patient advocate, negotiating with payors, agencies, and vendors to secure necessary resources and services.
- Clinical Competency: Demonstrate expertise in case management, utilization management, and performance improvement, with a strong understanding of pre-acute and post-acute levels of care.
- Communication and Collaboration: Develop and maintain effective relationships with patients, families, physicians, and other healthcare professionals to ensure seamless communication and coordination of care.
- Education: Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required, or demonstrated success in a CHRISTUS Care Manager I position for at least 5 years.
- Experience: Two or more years of clinical experience, with one year in the acute care setting preferred.
- Certifications: RN or LMSW in the state of employment required for new hires; LBSW accepted for associates with 5+ years of demonstrated success and experience in the CHRISTUS Care Manager I role.
