POSITION SUMMARY


The Registered Nurse is responsible for planning, organizing, and directing hospice services, bringing a wealth of nursing experience with a focus on community health education and practice.

This professional role leverages community resources to tailor services that address the unique needs of individuals and families in their homes and communities.

KEY RESPONSIBILITIES

Patient Care

  • Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs, including thorough physical assessments and medical histories.
  • Delivers professional nursing care by applying all components of the nursing process.
  • Evaluates and monitors patient status by:
A. Developing and initiating a care plan

B. Regularly reassessing patient and family/caregiver needs


C. Adjusting the care plan as necessary based on changes in patient status and needs.


  • Utilizes health assessment data to establish nursing diagnoses.
  • Creates a care plan with defined goals based on nursing diagnoses, incorporating palliative care actions, and involving patients and families in the planning process.
  • Implements appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed by the physician.
  • Provides counseling to patients and families to address nursing and related needs.
  • Offers health care instructions tailored to the patient’s assessment and care plan.
  • Assists patients with daily living activities, promoting self-sufficiency and optimal comfort care.
  • Acts as Case Manager when designated by the Clinical Supervisor, coordinating care for an assigned patient caseload.
Communication

  • Maintains accurate and relevant clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
  • Communicates effectively with physicians about patient needs and condition changes, obtaining necessary orders.
  • Collaborates with community health professionals to ensure cohesive care planning.
  • Educates patients and families on self-care techniques, providing medication, dietary, and other instructions as per physician orders, while seizing opportunities for health counseling.
  • Ensures a safe environment for patients.
  • Facilitates continuity of care by assisting patients, families, and team members.
  • Works alongside families and the hospice interdisciplinary team to address the emotional needs of patients and caregivers.
  • Participates in interdisciplinary team meetings.
Additional Responsibilities

  • Engages in on-call duties as outlined in the on-call policy.
  • Ensures the availability of necessary equipment and services.
  • Supervises ancillary staff and delegates responsibilities as needed.
  • Takes responsibility for personal growth and development, enhancing professional knowledge and skills through continuing education and training.
  • Completes assigned case responsibilities as requested.
  • Actively participates in quality assessment and performance improvement initiatives.
QUALIFICATIONS

  • Graduate of a nursing program accredited by the National League of Nursing.
  • Current licensure in the state and CPR certification.
  • Registered nurses must have at least one (1) year of professional nursing experience within the last three (3) years; or possess a baccalaureate degree in nursing from an accredited program and a current RN license. A minimum of two (2) years of experience, with at least one year in public health, home care, or hospice nursing, is preferred.
  • Management experience is not required, but supervision of hospice aides is expected.
  • Strong observational, verbal, and written communication skills, along with problem-solving and basic math skills; nursing skills as per competency checklist.
  • Ability to perform prolonged walking or standing, lift, position, and transfer patients, and manage supplies and equipment. Requires considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing are necessary for performing nursing skills.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
