We are seeking a compassionate and skilled Hospice Licensed Vocational Nurse (LVN) to join our team at AccentCare. As a Hospice LVN, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of end-of-life care.
Key Responsibilities- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop individualized care plans to address these needs.
- Collaborate with interdisciplinary teams, including physicians, social workers, and chaplains, to ensure comprehensive care and support for patients and their families.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with company policies and regulatory requirements.
- Participate in ongoing education and training to stay current on best practices in hospice care and to enhance your skills and knowledge.
- Graduated from an approved school of professional nursing.
- One year of experience as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), preferred.
- Licensed to practice as a practical nurse or vocational nurse in the state of agency operation.
- Valid CPR certification, preferred.
- Ability to travel to all business locations, as needed.
