Market Benchmarked Salaries: As a non-profit provider, we regularly conduct market benchmarking salary surveys to ensure our compensation remains competitive and fiscally responsible. Pay Range: $20.50-$28.35

Responsibilities:

  • Coordinate and deliver care to hospice patients.
  • Provide emotional support to patients and families during challenging times.
  • Collaborate with a team of healthcare professionals to ensure superior patient care.
  • Address personal care needs and promote patient comfort.
  • Participate in interdisciplinary meetings and organizational activities.
  • Document care and observe patient conditions.

Qualifications:

  • Preferred: Certified Home Health Aide issued by the State Department of Public Health.
  • Required: Certified Nursing Assistant Certificate issued by the State Department of Public Health.
  • Current BLS/CPR certification.
  • One year of aide and hospice experience preferred.
  • Valid California Driver's License and reliable transportation.
  • Strong observation, problem-solving, and communication skills.
  • Proficient in computer use; prior electronic medical record (EMR) experience preferred.
  • Willingness to work flexible hours, including evenings and weekends.
  • Reliable transportation for travel throughout the service area.
