JOB SUMMARY
The registered nurse is responsible for planning, organizing, and overseeing hospice care, with a focus on community health education and experience. The professional nurse utilizes community resources to design and implement services that address the needs of individuals and families in their homes and communities.

KEY RESPONSIBILITIES
Patient Care
1. Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs. Performs thorough physical assessments and reviews medical histories.
2. Delivers professional nursing care by applying all elements of the nursing process as outlined in the state Nurse Practice Act.
3. Evaluates patient status by:
A. Developing and initiating a care plan
B. Regularly reassessing patient and family/caregiver needs
C. Adjusting the care plan as necessary
4. Implements the care plan and modifies it as patient conditions and needs evolve.
5. Utilizes health assessment data to formulate nursing diagnoses.
6. Creates a care plan that sets goals based on nursing diagnoses and includes palliative nursing interventions, involving the patient and family in the planning process.
7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as directed by the physician.
8. Provides counseling to patients and families to address nursing and related needs.
9. Offers health care education to patients as needed based on assessments and care plans.
10. Assists patients with daily living activities and promotes their efforts towards self-sufficiency and optimal comfort care.
11. Acts as Case Manager when designated by the Clinical Supervisor, coordinating care for assigned patients.
Communication
1. Maintains accurate and relevant clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
2. Communicates with physicians regarding patient needs and reports any changes in condition; obtains necessary physician orders.
3. Collaborates with community health professionals to coordinate care plans.
4. Educates patients and families on self-care techniques as appropriate, providing instructions on medications, diet, and other care as prescribed by the physician.
5. Ensures a safe environment for patients.
6. Facilitates continuity of care with patients, families, and team members.
7. Works alongside families and the hospice interdisciplinary team to address the emotional needs of patients and families.
8. Participates in interdisciplinary team meetings.
Additional Responsibilities
1. Engages in on-call duties as outlined in the on-call policy.
2. Ensures necessary equipment and services are arranged.
3. Supervises ancillary staff and delegates responsibilities as needed.
4. Takes responsibility for personal growth and development, maintaining and enhancing professional knowledge and skills through continuing education and training.
5. Completes assigned case assignments as requested.
6. Actively participates in quality assessment and performance improvement initiatives.

QUALIFICATIONS
1. Graduate of a National League of Nursing accredited nursing program.
2. Current California nursing license and CPR certification.
3. Registered nurses must have a minimum of one (1) year of professional nursing experience within the last three (3) years or possess a baccalaureate degree in nursing from an accredited program and a current RN license. A minimum of two years' experience, with at least one year in public health, home care, or hospice nursing is preferred.
4. Must possess a valid driver's license and have access to a reliable, insured vehicle.
5. Management experience is not required; responsible for supervising hospice aides.
6. Self-motivated and capable of working independently.
7. Strong observational, verbal, and written communication skills, along with problem-solving abilities and basic math skills.
8. Physical ability to perform nursing tasks, including prolonged walking or standing, lifting, positioning, and transferring patients, as well as handling supplies and equipment.
