Job Summary

Blossom Ridge Home Health Agency LLC is seeking a skilled Weekend RN Coordinator to join our Hospice team in Sacramento. This role offers a fantastic opportunity to elevate your nursing career while working with a dedicated team.

Key Responsibilities
  • Oversee LVNs and HHAs to ensure high-quality patient care and adherence to agency policies, Medicare/Medicaid regulations, and Joint Commission Standards during the weekend shift.
  • Monitor staffing needs and assist in triaging patient needs during the weekend.
  • Perform initial home care patient visits and re-evaluate patients' needs and progress on a regular basis as needed.
  • Initiate the plan of care under doctors' orders and perform admission, transfer, re-certification, and death/discharge for hospice patients as needed.
  • Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
  • Coordinate patient services and provide additional support to the Intake team as needed.
  • Provide direct supervision of LVNs and HHAs and notify the physician and other IDG members of changes in patients' conditions.
  • Perform skills outlined in the agency's approved policy and procedure manual and update clinical records according to policy and procedures.
Requirements
  • Be a California registered nurse (R.N.) with a current license.
  • Have one year of prior professional nursing experience.
  • Have intermediate computer skills and knowledge of Medicare and Medicaid regulations and Joint Commission Standards.
  • Have one year of management or leadership experience.
  • Have a driver's license and reliable transportation and current automobile insurance.
