The registered nurse is responsible for planning, organizing, and overseeing hospice care, emphasizing community health education and experience. The professional nurse utilizes community resources to design and implement services that cater to the needs of individuals and families in their residences and communities.
Essential DutiesPatient Care:
- Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs, providing thorough physical evaluations and histories of current and past illnesses.
- Delivers professional nursing care by employing all aspects of the nursing process as defined by the state Nurse Practice Act.
- Evaluates patient status by:
- Creating and initiating a care plan
- Regularly reassessing patient and family/caregiver needs
- Modifying the care plan as necessary
- Implements the care plan and adjusts it as patient needs evolve.
- Utilizes health assessment data to establish nursing diagnoses.
- Develops a care plan with defined goals based on nursing diagnoses, involving the patient and family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing actions, administering medications and treatments as prescribed by the physician.
- Provides counseling to patients and families to address nursing and related needs.
- Offers health care guidance to patients as per assessment and care plan.
- Assists patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Acts as Case Manager when designated by the Clinical Supervisor, coordinating care for assigned patients.
Communication:
- Maintains accurate clinical notes regarding patient conditions and care provided, documenting pain and symptom management changes.
- Communicates with physicians regarding patient needs and condition changes, obtaining necessary orders.
- Collaborates with community health professionals to coordinate care plans.
- Educates patients and families on self-care techniques and provides medication and dietary instructions as directed by the physician.
- Ensures a safe environment for patients.
- Facilitates continuity of care with families, caregivers, and team members.
- Works alongside families and the hospice interdisciplinary team to address emotional needs.
- Participates in interdisciplinary meetings.
Additional Responsibilities:
- Participates in on-call duties as outlined in the on-call policy.
- Ensures necessary equipment and services are arranged.
- Supervises ancillary staff and delegates responsibilities as needed.
- Takes responsibility for personal growth and development, maintaining professional knowledge through continuing education.
- Engages in quality assessment and performance improvement initiatives.
- Graduate of a National League of Nursing accredited nursing program.
- Current licensure in California and CPR certification.
- Registered nurses must have at least one year of professional nursing experience within the last three years or possess a baccalaureate degree in nursing from an accredited program and a current RN license. A minimum of two years' experience, with at least one year in public health, home care, or hospice nursing, is preferred.
- Must possess a valid driver's license and have access to a properly insured vehicle.
- Management experience is not required, but supervision of hospice aides is expected.
- Self-motivated and capable of working independently.
- Strong observational, verbal, and written communication skills, along with problem-solving abilities.
- Physical ability to perform nursing tasks, including lifting and transferring patients.
