Job Summary

We are seeking a skilled Clinical Care Specialist to join our team at the Community Health Center in Stamford, Connecticut. As a key member of our healthcare team, you will provide high-quality primary nursing care to individuals and families throughout the lifespan.

Responsibilities
  • Provide comprehensive professional nursing care to patients under the planned care model.
  • Collaborate with primary care providers to identify needs for preventive care, evidence-based chronic disease care, and care coordination.
  • Provide general and specific health care education to patients.
  • Work with patients to identify and monitor self-management goals.
Requirements

To be successful as a Clinical Care Specialist, you should have:

  • A Bachelor of Science in Nursing (BSN) degree.
  • Expertise in electronic health records.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.
Compensation

The estimated salary for this position is $65,000 - $85,000 per year, based on experience and qualifications.
