Community Health Nursing Home Care Coordinator

We are seeking a skilled and detail-oriented Nursing Home Care Coordinator to join our team at Community Health Centers of the Rutland Region. This role plays a vital part in ensuring the seamless delivery of care to our patients.

As a Care Management Specialist, you will assist providers in managing workflow, perform various nursing and administrative duties, and provide safe and efficient care to patients.

The ideal candidate will have a current Vermont LPN license, MA, and Valid CPR Certification. Prior experience working in a nursing home is preferred but not required. Experience with electronic medical records and basic keyboarding skills are essential.

In this role, you will communicate with providers about new orders, obtain and record vital signs, pre-load new patients for admissions, update patient records, document nursing home data, and maintain regulatory visits. You will also address daily summary action reports and ensure patient records are accurate and up-to-date.

We offer a competitive salary range of $24.50-36.61 per hour for LPNs and $19.00-28.50 per hour for Medical Assistants.
