The Harm Reduction Program Coordinator plays a vital role in facilitating client intakes and managing non-clinical services at our Harm Reduction Center. This position is essential in addressing the social and structural determinants affecting our clients, aligning with FHCHC's mission and values. By adhering to harm reduction principles, the coordinator will adopt a recovery-oriented approach for all individuals seeking assistance.

Key Responsibilities

Reporting to the Harm Reduction Lead, the Harm Reduction Program Coordinator will conduct annual screenings for social determinants of health using validated tools for all clients. Responsibilities include, but are not limited to:
  • Collaborating with external organizations and service providers, including governmental agencies, to connect clients with suitable resources.
  • Creating and assessing shared care plans tailored to individual needs.
  • Linking patients with community resources to address social needs.
  • Supporting and monitoring the completion of essential health maintenance tasks (e.g., lab tests, annual check-ups) and chronic disease management (e.g., diabetes, asthma).
  • Conducting home visits as necessary.
  • Identifying obstacles to care that affect clients' treatment adherence.
  • Working in partnership with clinical teams to address the comprehensive needs of high-needs patients.
  • Participating in relevant training sessions as required.
  • Documenting client interactions, referrals, and services in the EPIC electronic health record while ensuring secure communication with team members and clinicians.
  • Maintaining strict compliance with deadlines for reports and documentation.
  • Coordinating essential services such as food, laundry, and hygiene facilities for clients at the Harm Reduction Center.
Qualifications
  • Bachelor's degree in a health-related field or equivalent experience, along with a valid driver's license and/or reliable transportation; experience in care coordination and proficiency in electronic health records is essential; bilingual proficiency in Spanish and English is required.

Physical Requirements

Physical Demands: This role requires walking, bending, sitting, standing, writing, reading, telephone communication, data entry, and retrieving medical records. Mental Demands: The ability to manage shifting priorities in potentially stressful situations is crucial. Manual Dexterity: Proficiency in using a keyboard and telephone is necessary.
