Position Objective
The primary goal is to connect clients with housing, educational, employment, vocational, spiritual, recreational, medical, and treatment resources. This role aims to decrease the number of individuals in New Haven County who are affected by mental health challenges and/or substance abuse that may lead to homelessness.

Qualifications

  • Bachelor's degree required; Master's degree preferred or a minimum of four years of experience in outreach or substance abuse case management.
  • Experience working with individuals experiencing homelessness and those with a history of mental health and substance abuse issues.
  • Familiarity with outreach practices, engagement techniques, basic entitlements, housing options, community resources, and mental health concerns.
  • Strong organizational and networking skills to facilitate connections to medical, mental health, substance abuse treatment, employment, and educational resources.
  • Cultural sensitivity and knowledge of Trauma-Informed and Gender-Specific care services are essential.
  • Proficient in computer use with the ability to learn new software applications.
  • A valid Driver's License and a clean driving record are required. Knowledge of Harm Reduction and PTSD is advantageous. Bilingual in English and Spanish is preferred.

Key Responsibilities

  • Conduct street outreach to engage individuals with histories of homelessness, mental illness, substance abuse, unemployment, and educational challenges who are currently disconnected from services.
  • Provide comprehensive case management services to individuals in various community settings, including shelters and soup kitchens.
  • Collaborate with other outreach and shelter case managers as necessary.
  • Participate in all required network and staff meetings; engage in regular supervision with the Transitional Programs Manager; maintain an up-to-date caseload review.
  • Work in conjunction with the CAN Entry and Exit process in the New Haven area.
  • Ensure all required documentation is completed accurately and promptly.
  • Input all necessary client data into the Case Worthy computer system in a complete and timely manner.
  • Perform other related duties as assigned.

Supervision: Reports to the Program Manager of Case Management Services

Compensation: $25.50-$27.50 per hour, plus comprehensive benefits
