Position Overview:
The selected candidate will serve as the on-site clinical leader at FHCHC's Harm Reduction Center.
This clinician will deliver high-quality medical services to individuals seeking assistance at the Harm Reduction Center, in alignment with the policies and mission of Fair Haven Community Health Care (FHCHC).
Emphasizing the principles of harm reduction, the clinician will adopt a recovery-oriented approach for all individuals seeking these services.
Key Responsibilities:
- Deliver primary medical care, including urgent and primary care, for adult patients.
- Prescribe medications for opioid use disorder (MOUD) and alcohol use disorder (MAUD) as clinically appropriate.
- Provide pre-exposure prophylaxis (PrEP) for HIV prevention as clinically indicated.
- Develop individualized care plans for each patient, encompassing medical history, physical examination, diagnosis, and suitable treatment or referral, including hospitalization if necessary.
- Utilize available resources for diagnosis and treatment, such as laboratory tests, in a cost-effective manner.
- Collaborate with other clinicians, patients' families, and support staff regarding patient treatment and care when necessary.
- Refer cases requiring specialist services while maintaining primary care responsibility to ensure continuity of care.
- Engage in FHCHC's Quality Improvement program and participate in other committees as needed, pursuing continuing education opportunities to enhance medical knowledge and ensure quality patient care.
- Perform additional duties as required by FHCHC to achieve the goal of delivering high-quality services and healthcare.
- Graduation from an accredited APRN or PA program; valid medical license in the state of Connecticut; willingness to serve on the active medical staff of affiliated hospitals; qualified in Basic Life Support; experience in prescribing MOUD, MAUD, and PrEP.
- Bilingual (English/Spanish) is preferred.

Category I - Increased Risk Position with potential exposure to hazardous waste and bloodborne pathogens in a clinical setting.
Physical Requirements:Physical Demands: Involves walking, bending, sitting, standing, writing, reading, telephone use, data entry, and pulling medical records.
Mental Demands: Ability to manage continuously changing priorities under potentially stressful conditions.
Manual Dexterity Required: Ability to use a keyboard and telephone.
