Role Summary

This position is integral to our high-demand program. Team members have distinct roles and responsibilities to expedite client participation in our services. The Community Outreach Advocate II will deliver direct support to clients by assessing their needs, providing safe sleep education, and distributing cribs, along with relevant resources and referrals. This role involves leading multiple community workshops both in-person and online, as well as participating in outreach initiatives to promote awareness about Sudden Unexpected Infant Death (SUID) and safe sleep practices. Data entry for client interactions and the ability to lift 20-40 lbs frequently are essential aspects of this role.

Key Responsibilities
  • Respond to community requests for cribs through telephone intake and client scheduling for workshops and home visits.
  • Engage with participants via phone, text, email, or home visits to facilitate workshop registration and provide necessary resources.
  • Conduct assessments to identify client needs.
  • Gain knowledge on SUID, breastfeeding, and other pertinent topics and community resources.
  • Lead 3 to 6 educational workshops monthly on safe sleep, both virtually and at various locations.
  • Assist in the preparation for workshops and home visits, including reminder communications and logistics for crib deliveries.
  • Deliver cribs to clients and partner organizations.
  • Conduct home visits to set up cribs and provide education on safe sleep and SIDS risk reduction.
  • Engage in health promotion by offering client education and distributing educational materials.
  • Maintain meticulous records of all client interactions and follow-ups.
  • Ensure client confidentiality and share information only with client consent.
  • Utilize MCC's client database and other computer systems for documentation and communication.
  • Participate in training sessions on maternal and child health topics.
Additional Responsibilities
  • Refer clients needing intensive follow-up to appropriate agencies.
  • Communicate regularly with supervisors and team members regarding work progress.
  • Engage in outreach to connect with pregnant women and caregivers.
  • Assist in organizing client events that celebrate caregivers.
  • Represent MCC at community meetings and events as directed.
  • Conduct follow-up calls with clients to assess service satisfaction.
  • Adhere to MCC's security protocols for handling sensitive information.
  • Attend staff meetings to support communication and goal alignment.
  • Train new advocates or students as needed.
  • Supervise students and volunteers.
  • Assume supervisory duties when necessary.
  • Complete special projects as assigned by supervisors.
  • Perform other duties as required within the scope of the position.
Qualifications
  • High School Diploma or equivalent is required.
  • Minimum of one year of experience in community outreach or customer service is preferred.
  • Experience with community health issues is beneficial.
  • Familiarity with maternal and child health issues is preferred.
  • Bilingual skills (Spanish, Arabic, Haitian Creole) are a plus.
  • A valid driver's license is required.
