POSITION SUMMARY:
The Elder Care Coordinator is dedicated to improving the quality of life for residents in senior living facilities. This role involves assessing individual service requirements, facilitating access to comprehensive service networks, and ensuring that each client receives effective and coordinated support. The Coordinator also encourages resident engagement in programs designed to promote their physical, social, and emotional health.
Key Responsibilities:
- Provide advocacy and assistance related to social and supportive services for all residents.
- Offer current information regarding programs such as Medicare, Medicaid, and other entitlements.
- Conduct outreach to new residents to foster a welcoming atmosphere and inform them of available services.
- Perform thorough assessments and periodic evaluations of residents requiring assistance, which may lead to personalized care management plans.
- Deliver counseling and support to residents and their families to resolve issues arising during the service coordination process.
- Establish and maintain connections with community resources to stay informed about available services that address resident needs.
- Identify and address crises, conflicts, and critical situations affecting residents, including protective services and mental health concerns.
- Recognize service gaps and develop innovative solutions to meet the needs of residents.
- Act as a liaison between residents and facility management, balancing advocacy for residents with the operational needs of the facility.
Qualifications:
Applicants should possess an Associate's degree in social work or a related field, along with at least one year of experience in the aging or human services sector. A combination of education and experience that provides knowledge of the human service system and experience working with older adults and individuals with disabilities is essential.
Additional Information:
Covid Vaccination is a requirement for employment. Candidates must be vaccinated prior to their start date unless a medical or religious exemption is approved.
