Mental Health Partnerships (MHP) is seeking a highly motivated and compassionate Community Outreach Coordinator to join our team. As a key member of our outreach team, you will play a vital role in connecting individuals experiencing homelessness or economic insecurity with vital community resources and services.
Key Responsibilities- Build Meaningful Relationships: Develop trusting relationships with participants to understand their unique needs and challenges.
- Identify and Access Resources: Assist participants in accessing community resources, including short-term housing, to increase their success in the community.
- Provide Crisis Intervention: Offer de-escalation and crisis intervention services as needed to ensure participant safety and well-being.
- Comply with MHP Policies: Maintain compliance with all MHP policies and procedures, reporting any known conduct that violates MHP policies or laws.
- Other Duties: Perform other duties as assigned to fulfill MHP's mission.
- Education: High School Diploma/GED required.
- Experience: 2 years of working with individuals experiencing homelessness, mental health conditions, and/or substance misuse.
- Credentials: Credentialed as a Certified Peer Specialist is a plus.
- Driver's License: Must possess a valid driver's license.
- Computer Skills: Basic computer skills, including desktop computing, email, time sheet management, electronic health records, and internet research.
- Ability to Work with Others: Ability to work with a partner and carry out assignments to completion within parameters of instructions given.
- Physical Demands: Ability to sit at a desk, lift up to 15 lbs, and perform essential job functions safely and successfully.
