Patient Care Coordinator

Position Overview: The Case Manager will oversee the integration of community resources to effectively address the medical, psychological, behavioral, and social requirements of patients. This role is crucial in identifying patients at risk and making informed decisions regarding their mental and physical health needs, including enhanced support from social work.
Key Responsibilities:
- Utilize clinical expertise in human development and psychological principles related to traumatic brain injuries, behavioral patterns, addiction, and PTSD.
- Comprehend the implications of traumatic brain injuries on both physical and psychological well-being and interpersonal relationships.
- Exhibit strong relationship-building capabilities across diverse cultural backgrounds.
- Demonstrate knowledge and experience in medical and mental health diagnoses, disabilities, and treatment methodologies, including acute, chronic, and traumatic conditions; familiarity with common medications and their effects; and proficiency in medical terminology. Mastery of the Diagnostic and Statistical Manual of the APA (DSM-5) is essential.
- Possess a thorough understanding of community resources, with the ability to make suitable referrals and coordinate services effectively.
- Hold a Master’s degree in Social Work, Counseling, or Rehabilitation Counseling from an accredited institution.
- Maintain a Licensed Master Social Worker or Licensed Professional Counselor or Certified Rehabilitation Counselor status in Louisiana (or the ability to obtain prior to employment).
- Have a minimum of three years of experience in direct patient care, case management, and psychosocial issues related to mild to moderate TBI and PTSD.
- Be a Licensed Clinical Social Worker or Rehabilitation Counselor with 2-3 years of direct experience in managing mild to moderate traumatic brain injuries.
- Possess experience working with active duty military personnel and/or veterans.
- Have a background in mental health rehabilitation and dealing with complex medical and neurological conditions.
