Position Summary
This role serves as a vital link between patients, their families, healthcare providers, and the multidisciplinary team. The primary focus is to facilitate communication and assist with care transitions, ensuring that patients receive comprehensive support throughout their healthcare journey.
Key Responsibilities
- Act as a liaison to ensure seamless transitions between various care settings.
- Build strong relationships with patients and their care teams to promote continuity of care.
- Manage complex patient cases, aiming to reduce the likelihood of readmissions.
- Collaborate with physicians to ensure clear communication regarding patient care plans.
- Prepare and implement post-discharge care strategies.
- Identify and address barriers to care, advocating for process improvements.
- Work with leadership to enhance the clinical experience for patients and referring physicians.
- Adapt communication styles to meet the needs of diverse patient populations.
- Perform additional related duties as required.
Education
Required: Diploma in nursing.
Preferred: Associate's or Bachelor's degree in nursing.
Experience
Required: Minimum of 2 years in nursing.
Certifications
Required: Valid RN license in the state of practice and Basic Life Support (BLS) certification from the American Heart Association.
Preferred: Certification in a clinical specialty area.
Skills and Abilities
- Proficient in computer systems and web-based applications.
- Strong verbal and written communication skills.
- Excellent conflict resolution abilities.
- Flexibility to work varied schedules and travel as needed.
This position requires the ability to exert moderate physical effort, including lifting and moving objects. There may be exposure to blood and bodily fluids, necessitating adherence to safety protocols.
