Overview

Join LHC Group as a Full-Time Patient Care Management Assistant.

We prefer candidates with prior experience in home health care.

The Patient Care Management Assistant plays a crucial role in supporting the Patient Care Manager by coordinating a dedicated team to ensure seamless, high-quality care for home health patients within the designated area. This position emphasizes the importance of having a skilled and informed team committed to delivering exceptional care efficiently, adhering to physician directives and regulatory standards.

Key Responsibilities

  • Manage incoming communications from field staff, healthcare providers, referral sources, and patients, documenting messages and orders as necessary.
  • Regularly check and report on the status of incoming faxes to the relevant personnel.
  • Facilitate communication by coordinating and sending necessary documents to physician offices.
  • Track pending laboratory results, ensuring timely follow-up and communicating outcomes to physicians and relevant team members.
  • Oversee the status of patients in hospital settings to guarantee effective care coordination.
  • Assist the Patient Care Manager in analyzing various software reports that enhance care coordination and utilization management, aiming for positive patient outcomes.
  • Support the Patient Care Manager with authorization processes for payer sources as required.
  • Conduct follow-ups based on health check calls or tele-monitoring alerts, ensuring effective communication with team members.
  • Prepare patient lists and relevant documentation for weekly case discussions.
  • Provide direct patient care as needed, in line with the scope of practice, care plans, and physician orders, while collaborating with other healthcare professionals.
  • Complete required training and courses as mandated by LHC Group and state regulations.
  • Assist in reviewing clinician documentation to ensure accuracy and completeness in submissions by field staff.
  • Participate in the agency's Episode Management process.
  • Perform additional duties as assigned.

Qualifications

License Requirements
  • Current CPR certification is mandatory.
  • Active LPN licensure in the state of practice is required.
  • Possession of a valid Driver's License, vehicle insurance, and access to reliable transportation.

State-Specific Requirements

  • For Louisiana: Must hold a current, unrestricted LPN Licensure and have at least one year of LPN experience.
