At Touro Infirmary, we're dedicated to delivering exceptional patient-centered care. As a Rehabilitation Case Manager PRN, you'll play a vital role in coordinating admissions, engaging with families, and ensuring optimal outcomes for our patients.

Key Responsibilities:

  • Admissions Coordination: Register patients, obtain necessary authorizations, and facilitate a seamless transition to our rehabilitation center.
  • Patient Advocacy: Assess patient needs, communicate effectively with the healthcare team, and provide comprehensive support throughout their stay.
  • Discharge Planning: Coordinate discharge planning, provide follow-up support, and ensure a smooth transition back to the community.

Qualifications and Skills:

  • Experience: Minimum 2 years of experience in Case Management, Rehabilitation, or a related field.
  • Education: Bachelor's Degree in Nursing, Social Work, or Allied Health Professions.
  • Skills: Proficiency in developing care plans, strong communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
