Nurse Care Coordinator

We are seeking a highly skilled and compassionate Nurse Care Coordinator to join our team at LCMC Health. As a key member of our clinical team, you will play a vital role in delivering exceptional patient care and ensuring seamless coordination of services.
Key Responsibilities- Patient Care Coordination: Prioritize, coordinate, and delegate all care touchpoints for patients, ensuring timely and effective delivery of services.
- Collaboration and Communication: Work closely with multidisciplinary team members to ensure appropriate direct patient care, maintain open communication, and address patient concerns.
- Safe Environment: Maintain and advocate for a safe environment for patients, families, and staff, adhering to Joint Commission infection control and environment of care requirements.
- Wound Prevention and Treatment: Monitor and execute wound prevention and treatment protocols, ensuring effective management of patient care.
- Continuity of Care: Ensure continuity of care, efficient throughput, consistent communications, and an exceptional experience for every patient.
- Education and Support: Initiate patient and family education using appropriate resources and continuum of care services, promoting informed decision-making and empowerment.
- Education and Licensure: Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
- Certifications: Current American Heart Association BLS certification.
