Position Overview
Location: Benson Cancer Center
This role acts as a vital link among patients, caregivers, healthcare professionals, and multidisciplinary teams, ensuring seamless communication and collaboration. The position is responsible for discussing alternative care options with patients and their families, while also assisting with discharge planning and facilitating transitions across the healthcare continuum.

Key Responsibilities
  • Ensure continuity of care by managing transitions between various care settings.
  • Build strong relationships with patients and their care teams to support ongoing treatment and follow-up.
  • Oversee complex patient cases to minimize the risk of readmission.
  • Collaborate with physicians to coordinate and communicate care plans effectively.
  • Prepare and reinforce post-discharge care plans.
  • Identify and address barriers to care to enhance patient navigation through the healthcare system.
  • Work with leadership to improve clinical processes for both patients and referring physicians.
  • Adapt communication and care approaches to meet the needs of diverse patient populations.
  • Perform additional related duties as required.

Qualifications
Education
Required: Registered nurse diploma.
Preferred: Associate's or bachelor's degree in nursing.

Experience
Required: Minimum of 2 years of nursing experience.

Certifications
Required: Current registered nurse (RN) license in the state of practice; Basic Life Support (BLS) certification from the American Heart Association.
Preferred: Certification in a clinical specialty area.

Skills and Abilities
  • Proficient in computer systems and web-based applications.
  • Strong verbal and written communication skills.
  • Excellent conflict resolution abilities.
  • Flexibility to work various shifts, including weekends and holidays.

Physical and Environmental Demands
This position may require lifting and moving objects of varying weights and involves exposure to blood and bodily fluids. The role is performed in a healthcare environment where there is a risk of exposure to communicable diseases and hazardous materials.
