Position Summary
This role serves as a vital link between patients, caregivers, healthcare providers, and multidisciplinary teams. The individual will engage in discussions regarding alternative care options and assist with discharge planning needs, ensuring a seamless transition along the healthcare continuum. The goal is to achieve quality and cost-effective outcomes through collaboration with the multidisciplinary team.

Essential Responsibilities
  • Facilitates continuity of care by ensuring smooth transitions between various care settings.
  • Builds relationships with patients and their healthcare teams to navigate subsequent treatments and follow-ups, reflecting a commitment to continuity of care.
  • Manages complex patient care scenarios to minimize the risk of readmission.
  • Collaborates closely with physicians to coordinate and communicate the patient's care plan, ensuring all team members are informed and engaged.
  • Prepares and reinforces post-discharge care plans to support patient recovery.
  • Identifies barriers to care and advocates for process improvements to enhance patient navigation through the healthcare system.
  • Works with leadership to review and enhance clinical processes for referred patients and their physicians.
  • Adapts communication and behavior to meet the needs of diverse patient populations, ensuring respect for privacy and individual preferences.
  • Performs additional related duties as necessary.

Qualifications
Education
Required: Registered nurse diploma.
Preferred: Associate's or bachelor's degree in nursing.

Experience
Required: Minimum of 2 years of nursing experience.

Certifications
Required: Current registered nurse (RN) license in the state of practice; Basic Life Support (BLS) certification from the American Heart Association.
Preferred: Certification in a clinical specialty area.

Knowledge, Skills, and Abilities
  • Proficient in computer systems, software, and web-based applications.
  • Strong verbal and written communication skills, capable of presenting information clearly to various audiences.
  • Excellent conflict resolution abilities.
  • Flexibility to work various schedules, including weekends and holidays, and travel between facilities as needed.

Physical and Environmental Demands
This position requires the ability to exert moderate physical effort, including lifting and moving objects. The role involves exposure to blood, body fluids, and potential communicable diseases. Employees may also encounter hazardous materials in a healthcare setting.
