Position Overview:

The Outreach Patient Care Coordinator plays a crucial role in supporting patients at Care Alliance Health Center, whether in a clinical setting or through outreach initiatives. This position focuses on identifying and mitigating obstacles to accessing healthcare services. Key responsibilities include assisting with healthcare navigation, facilitating benefits enrollment, implementing targeted interventions for specific populations, managing community casework, and advocating for patients. Additionally, the coordinator will assist in patient check-ins and follow up on referrals and care plans.

Key Responsibilities:

*Responsibilities include, but are not limited to:

· Manage appointment scheduling and maintain accurate records for patients across various environments, including outreach clinics and shelters.

· Evaluate patient eligibility for services, support enrollment and recertification processes, and coordinate with providers for specialty care outside of Care Alliance. Facilitate access to private benefits, including pharmaceutical assistance programs, Medicaid, Medicare, and other support services.

· Provide advocacy, assessment, planning, and case management for vulnerable populations dealing with chronic conditions and mental health issues. Educate patients on navigating new insurance options and understanding managed care networks.

· Enhance patient understanding of their health conditions, treatment plans, and next steps, ensuring connections to licensed clinical professionals as needed.

· Identify and address non-medical barriers to health and self-sufficiency, such as transportation, housing, and financial resources. Facilitate connections to community resources, including transportation assistance, housing support, and job training programs.

· Assist patients in scheduling appointments, coordinating insurance benefits, and retrieving necessary medical documentation and follow-up information.

· Build and maintain positive relationships with community organizations and social service agencies to effectively link patients to appropriate resources.

· Connect patients with specialists, such as legal aid or housing case managers, and ensure consistent follow-up with both patients and external providers.

· Conduct regular reviews of outstanding referrals, including pending appointments and specialty visit outcomes.

· Encourage patient engagement in primary care and provide assistance in completing necessary documentation for various agencies.

Qualifications:

Required:

· High School diploma with an associate degree or equivalent experience preferred; a bachelor's degree in social work, healthcare administration, or a related field is advantageous.

· 1-3 years of experience in case management, counseling, or working with vulnerable populations, including those with mental health or substance use challenges.

· Familiarity with electronic health records systems, such as EPIC, is a plus.

· Availability to work standard hours, with flexibility for nontraditional hours as needed.

· Strong knowledge of community resources and the ability to collaborate effectively with service providers.

· Capability to work independently and as part of a multidisciplinary team.

· Excellent problem-solving skills and the ability to manage paperwork efficiently while maintaining confidentiality.
