As a Patient Care Coordination Nurse, you will engage with a multidisciplinary team to support high-risk patients throughout their healthcare journey.

Your role will involve providing comprehensive care coordination and disease management for patients with chronic conditions or those recovering from surgical procedures. You will focus on patient outreach and care coordination to ensure optimal health outcomes, reduce unnecessary emergency department visits, and enhance patient satisfaction.

Responsibilities:
  • Identify patients requiring ongoing care coordination for their specific health conditions.
  • Assess the level of involvement needed from the specialty care team and coordinate with primary care providers.
  • Utilize assessment tools to identify patients with potential care needs.
  • Conduct targeted outreach to high-risk patients to ensure timely care delivery.
  • Employ technological resources to manage patient populations effectively.
  • Perform comprehensive clinical assessments addressing medical, behavioral, and social needs.
  • Collaborate with the healthcare team to establish goals and interventions to enhance patient outcomes.
  • Monitor patient adherence to care plans and make necessary adjustments.
  • Serve as the primary contact for patients regarding their care and facilitate access to services.
  • Assist in managing transitions of care and address barriers to receiving care.
  • Work alongside other care coordination teams to ensure comprehensive patient support.
  • Educate patients and families on self-management and health maintenance.
  • Oversee clinical staff in home care settings to ensure quality service delivery.

Qualifications:
  • Graduate from an accredited nursing program.
  • BSN preferred.
  • Current state licensure as a Registered Nurse (RN) required.
  • Specialty certification preferred.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Three to five years of nursing experience required.

Physical Requirements: This role requires full range of motion, manual dexterity, and the ability to work in a dynamic environment. Candidates must be able to lift light weights and may have exposure to communicable diseases.

Personal Protective Equipment: Adherence to standard precautions and use of personal protective equipment as necessary.
