Patient Care Advocate

Care Alliance Health Centers is seeking a dedicated Patient Care Coordinator to join our team and make a real difference in the lives of our patients.
As a Patient Care Coordinator, you will play a vital role in ensuring our patients receive comprehensive and coordinated care. You will work closely with patients to understand their needs, develop personalized service plans, and connect them with the resources they require to achieve optimal health outcomes.
Responsibilities:- Provide compassionate medical and social case management services to patients.
- Conduct thorough assessments of patients' psychosocial needs and create individualized service plans tailored to their unique circumstances.
- Offer ongoing support and coordination for patients managing chronic illnesses, empowering them to actively participate in their healthcare journey.
- Assist patients in adhering to their prescribed treatment plans and provide guidance on navigating the healthcare system.
- Connect patients with valuable educational opportunities and support groups, such as diabetes education programs and community resources.
- Facilitate seamless referrals and consultations both within our organization and with external healthcare providers.
- Ensure timely follow-up after scheduled appointments and referrals to monitor patient progress and address any concerns.
- Coordinate transportation assistance for patients attending medical appointments, removing barriers to accessing care.
- Cultivate strong relationships with healthcare professionals and community organizations to expand our network of support services.
- Assist patients in completing applications for prescription assistance programs and financial counseling for specialty services.
- Serve as a liaison between patients and collaborative healthcare services, ensuring smooth communication and coordination of care.
- Provide comprehensive information and referrals to individuals seeking supportive services, such as employment assistance, transportation options, county benefits, clothing, birth certificate acquisition, state identification cards, and furniture support.
- Build and maintain positive relationships with community resources and social service agencies to connect patients with a diverse range of services that promote self-sufficiency.
- High School Diploma, Associate's or Bachelor's degree in Public Health, Social Work, or a related field preferred.
- Minimum 2 years of experience as a Patient Navigator, Care Coordinator, or Case Manager.
- Proven experience in medical and/or resource coordination for at least 2 years.
- Familiarity with EPIC and the HUB systems is highly desirable.
- Experience working with vulnerable populations facing chronic illnesses, mental health challenges, and substance abuse issues.
- Demonstrated ability to work compassionately and effectively with a diverse patient population.
- Strong organizational skills, attention to detail, and proficiency in completing paperwork accurately and confidentially.
