Position Overview
As an RN Care Coordinator, you will be an integral part of our multidisciplinary team, focusing on high-risk patients. Your role will involve coordinating care and managing chronic conditions to improve patient outcomes and satisfaction.

Key Responsibilities:
  • Identify patients requiring ongoing care coordination for their specialty conditions.
  • Collaborate with the specialty care team to outline involvement and identify primary care teams.
  • Utilize assessment tools to determine patients' care needs.
  • Conduct outreach to high-risk patients to ensure timely care delivery.
  • Manage patient populations using technological tools.
  • Perform comprehensive clinical assessments, addressing medical, behavioral, and social needs.
  • Communicate care coordination plans to patients and families.
  • Work with interdisciplinary teams to set goals and develop intervention plans.
  • Monitor patient adherence to care plans and reassess progress.
  • Close care gaps related to chronic diseases and surgical episodes.
  • Serve as the primary contact for patients regarding their care.
  • Facilitate transitions of care and address barriers to receiving services.
  • Collaborate with other care coordinator teams to enhance patient support.
  • Educate patients and families on self-management and health maintenance.

Qualifications:
  • Graduate from an accredited nursing program; BSN preferred.
  • Current state licensure as a Registered Nurse (RN).
  • Experience in nursing, preferably three to five years.
  • Strong critical thinking and decision-making skills.

Physical Requirements:
  • Ability to perform a full range of motion and manual dexterity.
  • May require exposure to communicable diseases.
