Position Overview
As an RN Care Coordinator within the Cerebrovascular Center, you will be integral to our outpatient care team. This role offers a unique opportunity to build meaningful relationships with patients, guiding them through their healthcare journey. You will not only manage care and provide medical guidance but also offer comfort and support as patients navigate their health challenges.

Key Responsibilities
  • Identify patients requiring ongoing care coordination for specialty conditions.
  • Assess the involvement needed from the specialty care team and coordinate with primary care providers.
  • Utilize assessment skills and tools to recognize patients with potential care needs.
  • Engage in proactive outreach to high-risk patients to ensure efficient care delivery.
  • Leverage technology to manage patient populations effectively.
  • Conduct thorough clinical assessments, considering medical, behavioral, and social needs.
  • Communicate care coordination details to patients and families, ensuring transparency.
  • Collaborate with interdisciplinary teams to set goals and develop care plans.
  • Monitor patient adherence to care plans and adjust as necessary.
  • Serve as the primary contact for patients regarding their care and facilitate access to services.
  • Assist in managing transitions of care and address barriers to receiving care.
  • Work alongside other care coordinators to ensure comprehensive patient support.

Qualifications
  • Graduation from an accredited nursing program; BSN preferred.
  • Current state licensure as a Registered Nurse (RN).
  • Experience in ambulatory care is advantageous.
  • Strong communication skills and a compassionate approach to patient care.
  • Ability to work independently and manage multiple priorities effectively.
