Position Overview

As a Care Coordination Nurse within the Cerebrovascular Center, you will be an integral part of our outpatient care team. This role allows you to build meaningful relationships with patients, guiding them through their healthcare journey. Your responsibilities will include:
  • Coordinating care and offering medical guidance while providing emotional support to patients facing health challenges.
  • Collaborating with a multidisciplinary team to ensure comprehensive care for high-risk patients.
  • Managing care for patients with chronic conditions or those requiring surgical intervention, focusing on outreach and coordination to achieve optimal health outcomes.

Key Responsibilities
  • Identify patients needing ongoing care coordination for their specific health conditions.
  • Assess patient needs using clinical evaluation tools and conduct outreach to high-risk individuals.
  • Utilize technology to manage patient populations effectively.
  • Conduct thorough clinical assessments addressing various patient needs.
  • Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.

Qualifications
  • Registered Nurse (RN) with experience in an outpatient setting.
  • Strong interpersonal skills and a compassionate approach to patient care.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
