Position Overview
As a RN Care Coordinator within the Surgical Epilepsy Center, you will be part of a renowned institution recognized for its exceptional patient care and cutting-edge research. This role is pivotal in ensuring high-risk patients receive comprehensive care and support throughout their healthcare journey.

Key Responsibilities
  • Identify patients requiring ongoing care coordination for their specialty conditions.
  • Collaborate with the specialty care team to outline involvement and identify primary care teams.
  • Utilize assessment tools to determine patient care needs and coordinate outreach for high-risk patients.
  • Conduct thorough clinical assessments, considering medical, behavioral, and social needs.
  • Work with interdisciplinary teams to establish goals and interventions aimed at improving patient outcomes.
  • Monitor patient adherence to care plans and adjust as necessary to ensure optimal health management.
  • Serve as the primary contact for patients and families, facilitating communication and addressing concerns.
  • Assist in managing transitions of care, ensuring effective communication across various healthcare settings.
  • Engage with other care coordination teams to enhance patient support and resource access.

Qualifications
  • Current state licensure as a Registered Nurse (RN).
  • Experience in nursing, preferably in a care coordination or specialty setting.
  • Strong communication and critical thinking skills.
  • Ability to work independently and collaboratively within a team.

Education
  • Graduate from an accredited nursing program; BSN preferred.
