POSITION SUMMARY:
A Clinical Nutrition Specialist in training, with less than one year of clinical experience, will collaborate closely with a certified dietitian. Under the direct oversight of an experienced renal dietitian, this role will deliver nutritional services to both in-center and home patients, aiming to acquire the necessary experience mandated by CMS until the completion of the required clinical experience to qualify as a dietitian.
This position actively engages in process enhancement initiatives to improve the likelihood that patients meet their individualized health goals as established by their healthcare provider.

KEY RESPONSIBILITIES:

• Uphold the company's values and customer service standards.

• Ensure exceptional service to all internal and external stakeholders.

• Foster and maintain effective relationships through timely and clear communication.

• Proactively address and resolve customer service concerns in a timely manner.

• Calculate dietary prescriptions in accordance with established standards.

• Assess patients' understanding of dietary needs related to kidney health.

• Complete Nutrition History forms to assist CMS-qualified dietitians in conducting assessments.

• Provide tailored educational resources to patients, families, and caregivers regarding nutritional status and relevant lab results, considering individual learning preferences.

• Analyze nutrition-related lab results, counsel patients and caregivers, and develop actionable strategies to support optimal nutritional health.
Communicate lab findings and recommended treatment adjustments to the interdisciplinary team as needed.
Engage with physicians and facility staff regarding lab results when appropriate.
Monitor patient adherence to nutritional therapy and address any barriers to achieving health goals.

ADDITIONAL DUTIES:

• Review each patient's albumin levels and body weight monthly, identifying malnourished individuals and those at risk, collaborating with the interdisciplinary team to determine suitable interventions.

• When necessary, recommend patients for nutritional supplements to their physicians as per disease management protocols.

• Assist in managing patients' fluid and volume requirements.

• Provide education on fluid and sodium management as appropriate.

• Collaborate with the interdisciplinary team on managing mineral metabolism, renal bone health, anemia, and overall nutritional status.

• Participate in Quality Assessment and Performance Improvement meetings alongside a CMS-qualified dietitian.

• Attend relevant staff meetings as required.

• Ensure a safe and clean working environment in collaboration with the interdisciplinary team.

• Engage in pharmacy-related responsibilities, including setup and management.

• Educate patients on emergency preparedness concerning food and nutrition.

• Conduct staff training sessions as needed.

• Adhere to company protocols and medical record policies.

• Maintain and enhance professional knowledge and skills for effective practice.

• Keep dietetic registration and continuing education hours up to date as per American Dietetic Association and state regulations.

WORK ENVIRONMENT:

• The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels. Exposure to infectious materials may occur.

• Daily responsibilities include desk work, computer use, and interaction with patients and healthcare staff.

• Travel between assigned facilities and community locations may be required for training and meetings.

EDUCATION AND CREDENTIALS:

• Bachelor's degree in Nutrition or a related field; Master's degree preferred.

• Registered Dietitian credential from the Commission on Dietetic Registration.

• Current state licensure as applicable.

EXPERIENCE:

• Less than one year of clinical experience is required.
