Role Summary

The Reentry Support Specialist plays a crucial role in assisting program participants in crafting housing stability strategies and overcoming obstacles to secure housing. This position is integral to our mission of preventing and ending homelessness.

We serve a diverse population, including adults, veterans, families, and young adults across various locations in Santa Clara County and southern San Mateo County.

Key Responsibilities
  • Manage a caseload of 20-30 participants, providing comprehensive case management services.
  • Conduct initial eligibility assessments and ongoing evaluations of participant needs.
  • Assist participants with budgeting, financial assistance administration, and resource referrals.
  • Implement crisis de-escalation techniques and develop safety plans.
  • Engage in collaborative care coordination and case conferencing.
  • Ensure compliance with all agency and contractual obligations.
  • Maintain accurate documentation of participant interactions and services provided.
  • Participate in training and professional development opportunities.

Qualifications

Experience and Skills
  • Minimum of 1 year of experience in social work or related fields.
  • Bachelor's degree in Social Work or a related discipline is preferred but not mandatory.
  • Proficient in Microsoft Office applications.
  • Valid driver's license and reliable transportation required.
